Attendance:

I am required to track student attendance in this course.  Students can be dropped if they miss more than 5 class periods.

You are required to do the Tutorial Examples on the course calendar prior to coming to class. You must bring your notes from the tutorials for that day to class. There will be a quiz directly from your tutorial notes EVERY day the first 5 minutes of class (you must use your notes). If you are more than 4 minutes late, you will be counted as tardy and cannot take the quiz. 2 tardies will equal 1 absence.

I will take roll from the quiz papers every day. Even if you did not look at the tutorials, you must hand in a blank paper with your name on it so I can take roll. If you arrive more than 15 minutes late, then you will be counted as absent..

Tests may NOT be made up if you are absent.
Online quizzes WILL close every Tuesday at 11:59pm and will not be re-opened for any reason. If you do not take a Tuesday quiz, then you will be counted as absent that day.  You can always take the online quizzes early, but not late.
If you know ahead of time you will miss a test, you must bring me your excuse at least 48 hours in advance, and arrange to take the test EARLY. Refer to the student handbook for absences counted as “official” (Please do not bring in a doctor's note, as that is NOT considered an excused absence)   Please make every effort possible to be on time for class, and do not plan to leave class early, as this is very disruptive to the class. 15 minutes on either end will count as an absence. The instructor reserves the right to ask you to leave the class if you are disruptive in any way, and it will count as an absence. The procedure for disruptive behavior in the Student Handbook will be followed before any student will be allowed back in the class.  

If you wish to drop the class at any time, it is your responsibility to take the necessary actions.  Any student wishing to drop this class after the 45th day MUST have my signature on the drop form and will receive a “Y” if they are failing at that time.   The instructor reserves the right to refuse to drop a student the last 2 weeks of class.  I will NOT allow anyone to drop during Final’s week.

At the beginning of each course, each faculty member will provide students with written attendance requirements. It is the student's responsibility to consult with the instructor regarding official or unofficial absences. Absences begin to accumulate with the first scheduled class meeting.

• Students bear the responsibility of notifying the Admissions and Records Office/Office of Student Enrollment Services when they discontinue studies in a course or at the college. Please refer to Appendix S-7 for Withdrawal Procedures.

A. OfficialAbsences
1. Official absences are those which occur when students are involved in an official activity of the college (e.g., field trips, tournaments, athletic events) and present an official absence excuse form. Absences for such events shall not count against the number of absences allowed by an instructor or department. Students who must miss a class for an official reason must obtain an official absence verification card from the appropriate vice president or designee, and present it to the appropriate instructor(s) before the absence. Prior arrangements must be made with each instructor for make-up work. If prior arrangements have been made, the student
will not be penalized.

2. Other official absences include jury duty and subpoenas. Appropriate documentation will be required. Prior arrangements must be made with each instructor for makeup work. If prior arrangements have been made, the student will not be penalized.

3. In the event of the death of an immediate family member, absences for periods of up to one week will not be counted against the number of absences allowed by an instructor or department. Students should contact instructor(s) as soon as possible to arrange for make-up work. Appropriate documentation will be required (for example, a copy of the obituary or funeral program). In specialized programs that require clinical rotations, this regulation may not apply.
